REGULAR ADMISSIONS

In order to properly enroll your child, you will need the following documents:

 

1. State of Florida health Examination form HRS-H 3040 yellow form including Tuberculin test.

2. State of Florida certificate of immunization HRS-680.

3. Original birth certificate, passport, certificate of arrival in the U.S.

4. Student school records or report card.

5. Proof of address must provide two of the following:

a. Properly executed lease agreement or broker's or attorney's statement of parents purchase of residence.

b. Electric deposit receipt or electric bill showing name and service address.

 

Registration requirements from out-of-county, state, country, and private schools:

Transfer from another Miami-Dade County Public School:

 

In order to properly enroll your child, you will need the following documents:

 

1. Parent or legal guardian must bring a withdrawal slip from sending school with proper picture I.D.

2. Proof of address in name of parent/guardian. 

 

 

MAGNET PROGRAM

ADMISSIONS

Applications to magnet programs/schools are open to all school-age children who either attend or are eligible to attend a Miami-Dade County Public School. Hialeah Middle School requires that:
 
1. Students must have and maintain a cumulative GPA of 2.0 or higher in core subject areas.
2. Students must have and maintain a cumulative GPA of 2.0 or higher in conduct.
3. Students who have ten (10) or more unexcused absences/tardies may be excluded.
 
Admission to the magnet program is determined by a performance audition or a portfolio review. 
 
Applications may be obtained online at
http://hms.dadeschools.net, or by contacting
the school at 305-681-3527.
 
Parents must contact the school directly for entrance requirements and to schedule an audition/portfolio interview. Transportation is limited and may not be available to all students.
 
The magnet lead teacher can assist in determining if a student is eligible for transportation.
